BEC Fiber Equipment Return Form (Required with equipment returns)
E911 Waiver (Required form for VoIP Phone Service)
BEC Letter of Agency Document (LOA) (Required form if keeping existing phone number with VoIP Phone Service. The LOA is a document that authorizes BEC Fiber to communicate with the consumers current telephone service provider and act on the consumers behalf in order to transition their current telephone number(s) to BEC Fiber’s network. This document is only required if you intend to keep your current home phone number with your BEC Fiber services.)
